The breast is the universal symbol of femininity, nurturing, love, and sexuality. Most women interested in breast augmentation or enlargement are intelligent, successful and well-adjusted individuals who are concerned with inadequate breast size. Reconstruction of a breast that has been removed due to cancer or other disease is one of the most rewarding surgical procedures available today. New medical techniques and devices have made it possible for surgeons to create a breast that can come close in form and appearance to matching a natural breast. Frequently, reconstruction is possible immediately following breast removal (mastectomy), so the patient wakes up with a breast mound already in place, having been spared the experience of seeing herself with no breast at all. Alternatively, the reconstruction can be performed weeks or even years after a mastectomy. There are essentially two different methods of reconstruction: Implants: The first method usually involves placing an inflatable device, or tissue expander, under chest skin and muscle, though there is growing evidence that not all tissue expanders need be placed under the muscle. Over a period of months, this device is slowly expanded to the desired size and shape by periodically injecting saline solution during office visits. The breast skin is typically insensate after mastectomy and therefore this is a painless procedure. Once fully expanded, the device is left in place for about 12 weeks until the scar tissue has matured and then the device is removed during a second short operation and replaced with a permanent implant. In some instances, particularly during nipple-sparing mastectomies, some patients are candidates for Direct-to-Implant (DTI) reconstruction and can therefore avoid additional surgery. The second method of breast reconstruction involves creating a new breast using the patient’s own tissue borrowed from elsewhere in the body. The most common area to borrow from is the abdomen, from which skin, fat, and muscle are removed and transferred to the chest to create a new breast mound. A side benefit to this procedure, called a TRAM flap, is that the patient gets a tummy tuck! Another advantage to the TRAM flap procedure, though recovery is significantly longer, is usually a breast that looks and feels more natural than one reconstructed with implants. After the new breast shape is obtained, additional procedures can be performed, including nipple reconstruction and lifting the opposite breast to achieve symmetry.

Inverted nipples are a common problem in women. They often look like a slit or hole on the nipple. Inverted nipples are often a source of self-consciousness and embarrassment. They can also cause frustrating problems with breastfeeding, infections, or other discomfort. Most people don’t know that this is a readily correctable problem. The cause of nipple inversion is usually a congenital shortening of one or more milk ducts that run from the breast tissue to the nipple. Sometimes the shortening is caused by an infection or previous surgery, but most of the time the patient is born with this problem. This short duct tethers the nipple and pulls it inward. There are many types of described repairs. More than likely the correction will involve division of the shortened duct(s) through a tiny incision located at the junction of the nipple and the areola. This is done under local anesthesia in the office. There is minimal pain and swelling. Patient often return to work the next day. The inverted nipple repair restores the nipple to a more natural, projecting appearance. Breastfeeding is not likely after repair. Sensation is usually not effected.

Abdominoplasty, also known as a tummy tuck, is the ideal surgical procedure for women and men who are bothered by the loose and saggy appearance of their stomachs. Tummy Tuck Candidates The typical patient that presents for abdominal contouring complains of extra skin, extra fat, a stretched or loose abdominal wall(diastasis), stretch marks, and/or poor lower abdominal skin quality. During the abdominoplasty operation, these muscles are plicated, or tightened, with sutures. The rectus muscles are visibly closer together and have better form. Usually, this results in removal of all of the skin and fat from just above the belly button down to the incision. This discarded skin tends to contain most or all of the stretch marks and often is inelastic and of poor quality. The upper abdominal skin, which is usually of better quality, more elastic and without stretch marks, is stretched to the lower incision. A body lift operation is a procedure that removes excess skin and fat from the sides and back of the hips, thighs and buttocks. Many patients that undergo a lower body lift have had significant weight loss, either through gastric bypass surgery, pregnancies, or through diet and exercise. After weight loss, the stretched skin envelope that once held the fat is loose and sagging. A body lift removes this excess and can dramatically improve the contours of the entire lower body. The procedure is done by making a horizontal incision along the waistline from the side of the lower abdomen all the way to the middle of the lower back. The skin and fat are elevated off of the thighs, hips, and buttocks and then “pulled up” like a loose pair of pants. The excess skin is removed and the incisions are closed, leaving the final horizontal scar within the pantyline. The procedure is then repeated on the other side. Results are often dramatic with a significant lift of the thighs and buttocks. The procedure is often combined with other procedures, especially liposuction. It is often done as a staged procedure following abdominoplasty, or tummy tuck. Lower Body Lift Pregnancy, weight gain/loss and time can cause many changes in the body. Women develop laxity along their body down the center midline (treated with abdominoplasty) and along the outer rear portion of the thighs and buttocks. These patient will often choose a lower body lift or buttocks lift. The procedures are similar and we choose on or the other based on the patient exam and desires. The typical patient that presents for buttocks contouring complains of extra skin, extra fat, not enough fat, stretch marks, and/or drooping of the buttocks and upper lateral thigh. Some patients complain that they have a flat droopy buttock and that their clothes don’t fit properly. Most report flabby skin that is dimpled or hangs (cellulite). The buttocks lift procedure addresses these types of issues providing a more attractive shape and contour of the buttocks and upper thighs. The buttocks lift is completed through an incision made across the upper buttock in an area that is usually covered by undergarments. Typically, some liposuction is done in the lower back or love handle region as well as the lateral thighs. The fatty tissue in the buttock is often preserved redistributed in the buttocks to improve the shape. No implant is necessary. Excess skin is removed, lifting the buttocks upwards. In the end, the love handles and lateral thighs are much smaller, the buttocks are higher and tighter, and there is more definition and shape to the buttocks. If you have more significant laxity of the lateral thighs, then the buttock lift incision can be extended and a lower body lift can be performed. Are you tired of your cankles? Cankles is a slang term that describes the lack of definition of the calf and ankle due to excess fatty tissue in the lower leg. Extra fatty tissue on the calf and ankle region can be due to being overweight but can also be genetic. While exercise is generally a great idea, you cannot spot reduce fat, especially in this area. Liposuction of the calves and ankles can reduce and eliminate the excess fatty tissue. Typically, the patient is left with a highly contoured lower leg that appears much more muscular, curvy and feminine. The procedure is done through small incisions made around the ankle and knee. Many patients come to me with concerns about facial aging, but when I bring up surgery, they become hesitant towards discussing it. Common worries they express are: Do I really need a face-lift? Will my face look pulled? Is a facelift a big, scary procedure? What if it looks unnatural or something goes wrong? The reality is if you want a more youthful looking face and to get rid of a saggy neck, jowls, fat loss, excess lid skin, droopy brows and wrinkles, surgery is your best option. There is no needle or laser that will fix those issues. But…there is hope for natural, beautiful results that you will love. What really happens to your face as you age? Skin: The sun, aging and genetics contribute to sun damage, brown spots, wrinkles, and other skin damage on the surface of the skin. Forehead/Eyelids: Some patients (and many surgeons) erroneously believe that the bulging fat in the lower eyelid is extra fat that should be removed. It’s not. This fat was just uncovered by fat loss in the cheek (like when low tide reveals things hiding in shallow water). Youthful brows start low medially and end high laterally (look at young models in magazines). Over time, the lateral portion of the brow rotates downward. As our brow descends, our eyelids send signals to our forehead and stimulate our forehead to raise our brow, which in turns leads to more wrinkles. This interaction is automatic and many times a low brow cannot be appreciated until the eyes are closed, which allows the forehead to relax. Mid-face: Young faces aren’t tight. They have fullness and curves (curves and tension are enemies). As we age, we lose facial fat, especially below the eyes in the mid-face area. The lid-cheek junction, which is about 5-6 mm below the lashes when young, moves downward so that it appears 15-20 mm below the lashes and forms a “bag”. The cheek deflates and falls, creating a jowl and disrupting the smooth contour of a youthful jawline. Neck: The neck skin loses elasticity and the thin neck muscles (platysma) develop banding. What do I do to create a more youthful face? When I think of facial rejuvenation, I think about the whole face and work backwards. A complete face at my practice involves doing the face, neck, upper and lower lids, brow, fat grafting and laser. I evaluate and consult with the patient for each procedure and eliminate what is unnecessary or undesired and together we create a plan to achieve your desired results. Some patients move forward with the complete face package and others get portions of it. How is a facial surgery at BPS different from other traditional facial surgeries? Traditional facelifts have unacceptably long scars and remove too much skin, which makes people look unnatural and too tight. Brow-lifts can unnaturally elevate the entire brow, which makes people look surprised. Fat is too often removed from the lower-lids, which makes them look even more sunken. Skin is pulled tight in an attempt to counter deflation rather than filling the lost volume. Lasers are used improperly and change the basic color of the skin and create scarring, leaving a look that is too white and waxy. Here is how I approach each area of the face: Face & Neck: On face and neck lifts, I use a short scar that starts in the sideburn area and ends up a short distance behind the ear lobe. I do not create a “hairline” scar, as that scar is a visible sign of surgery, which we are trying to avoid. (we need a photo of incision. The yellow incision represents a “short scar, the red represents the additional component of a “long scar”.) I often replace lost volume in the face with fat grafting and I rotate the cheek upwards rather than pull it backwards, for a more natural look. This restores the cheekbone and improves the lower lid areas while narrowing the jowl area (young face = apple, old face = pear). The neck has fat removed both on top of and below the muscles. The muscles are repaired in the midline and advanced on the sides (as the cheek is elevated) to create a tighter neck (tension is OK on the neck, a cylinder). The end result is a youthful, beautiful and natural appearance with minimal visible scaring. Lower Lids: When it comes to lower lids, one procedure doesn’t fit all. The aging in the lower lid can be due to fat loss, skin laxity, wrinkles, texture, spots, etc. I tailor the lower-lid procedure to the identified problems. Sometimes fat grafting and laser is enough. I may remove some excess skin or rearrange fat so it doesn’t look like it’s bulging. Often times I use a combination of procedures (lift cheek, fat graft, remove skin excess and laser) to achieve our desired results. Upper Lids & Brows: Upper lids and brows are evaluated together. For the brow, I use a small incision about 1-2 cm behind the hairline (hidden) to elevate the lateral part of the brow, giving it a natural and youthful appearance.

Prominent ears can do tremendous damage to one’s self-esteem. Teasing and name-calling during childhood can be devastating. Ear surgery, or otoplasty, is usually done to set prominent ears back closer to the head or to reduce the size of large ears. There can be one or more reasons why the ear is prominent. Often there is a weak or missing fold of cartilage in the ear (anti-helical fold). This causes the outer part of the ear to stick out, rather than back. In addition, the bowl part of the ear may be large or stick out. During surgery, we create a nice fold in the ear and/or rotate the bowl towards the head.
